In 1999, the Population Health Research group at the Alberta Cancer Board began to explore the feasibility of constructing a prospective population-based cohort in Alberta. There were two underlying themes in the development of the cohort concept: the creation of a population health laboratory (i.e. infrastructure) for the study of cancer (and other chronic disease) etiology, and the maximization of both short-term and long-term potential benefits for researchers in Alberta and beyond.
Between 2000 and 2005, a series of studies was undertaken to ascertain the feasibility of approaches for identifying and enrolling participants into a long-term cancer cohort study, collecting and processing baseline data, conducting follow-up surveys and obtaining blood samples. These studies demonstrated the feasibility of setting up a prospective cohort for the study of cancer/chronic disease etiology within a Canadian context, and laid a solid framework which has allowed us to expand the size and scope of the cohort resource within Alberta.
Furthermore, the development of collaborations with researchers in British Columbia, Quebec, Ontario and the Atlantic provinces has resulted in the receipt of funding to establish a pan-Canadian cohort, known as the Canadian Partnership for Tomorrow Project (CPT Project).
The principal objective of The Tomorrow Project is to develop a long-term cohort study that will act as a research platform to facilitate research into how various aspects of lifestyle, modifiable behaviours, environmental and genetic factors interact to influence risk of cancer, and potentially, other chronic diseases.

Recruitment procedures
Originally, a two-stage sampling design was used to identify eligible individuals. The first stage used a telephone-based random digit dial (RDD) procedure to select households from each of the regional health authorities in the province of Alberta. The second stage selected one eligible adult within each household. A household was defined as one or more persons, related or otherwise, who occupy the same private or collective dwelling. In households with more than one potential study participant, the person with the most recent birthday was selected for possible enrolment. In recent months, volunteers have been accepted into the project. Eligible adults who provide contact details (name, mailing address) are mailed a recruitment package that includes details regarding the study, a consent form and questionnaires concerning health, lifestyle, diet and physical activity. Those who complete and return a consent form and questionnaires are considered to be enrolled in the cohort.
In the CPT Project phase, participants are also invited to attend a study centre in order to provide blood/urine samples and to have a set of physical measurements taken.

Collection procedures
As a first phase, a self-administered baseline health and lifestyle questionnaire (HLQ) and a detailed consent form were sent by regular mail to individuals interested in study enrolment. Participants were classified as enrolled if they completed and returned the HLQ and the consent form. Approximately three months after enrolment into the study, two additional questionnaires concerning habitual diet and past year physical activity were mailed to participants. References for these instruments are provided at the end of this form.
Participants were asked if they would be willing to consider providing a blood sample for study purposes, should they be asked for one in the future. Also, participants were invited to provide their personal health number (PHN) and written consent for authorization for data linkage with the Alberta Cancer Registry, and with files held by Alberta Health and Wellness regarding health care utilization.
All questionnaires used in The Tomorrow Project have been mailed out to participants for self completion. Completed questionnaires are sent back to our office by mail, in pre-paid envelopes, and data entry is done by optical scanning, using the Cardiff TeleForm system.
In the CPT Project phase of the Tomorrow Project, two self-administered paper based questionnaires are completed by participants. These are the Update Health and Lifestyle Questionnaire (UHLQ) and the Physical Activity and Nutrition Survey (PANS). Participants are invited to complete these questionnaires at home, prior to coming to a study centre. At the study centre, the questionnaires are reviewed by a project assistant for completeness and clarity. In addition, the participant is asked to complete a short study centre questionnaire which is designed to capture information on fasting status, last use of tobacco and alcohol, use of medications and supplements in the 72 hours prior to the study centre visit, and other information that is required to determine eligibility for the blood draw, urine sampling and each of the physical measures. Following that step, the participant is invited to undergo a series of physical measures (see below). Blood (50ml) is then drawn by a qualified phlebotomist, and the participant is directed to a private washroom to provide a urine sample.
Follow up procedures
It is the aim of The Tomorrow Project to obtain follow-up and new information from participants at regular intervals during their participation in the study. Our second follow-up was administered in mid 2008. In late 2008, we started collecting anthropometric and physical measures, as well as updated health and lifestyle information, as part of the CPT phase of the Tomorrow Project.
It is anticipated that all participants will be contacted every three to four years following enrolment. At each time, they will be asked to complete self-administered questionnaires to provide updated health information. In addition, they will be asked to provide new information about exposures or personal history that may impact on future risk of cancer or other chronic disease, thereby building up a rich data repository. It is our intent to complete at least one follow-up round of direct physical measures (i.e. height, weight, waist/hip circumferences, body composition etc) with as many participants as possible, and also to collect/bank a second set of biological samples.
In addition to the active follow-up described above, we will also undertake passive follow-up by performing periodic linkage with administrative datasets.

These participants are being invited to reconsent to take part in the pan-Canadian cohort (Canadian Partnership for Tomorrow Project). A further 20,000 new participants will be invited to join the cohort in order to reach our target number.
