Funding agencies
The OHS is a joint project of Cancer Care Ontario, the Ontario Institute for Cancer Research, and the Canadian Partnership Against Cancer that, between 2009 and 2012, will recruit 150,000 Ontarians aged 35-69, who will be followed for 20 or more years. Data collected in the OHS include information about individuals, their environment and their communities, and will encompass the cultural, ethnic and geographic diversity of the province. The purpose of the study is to better understand the risk factors for and causes of cancer and other chronic diseases. The OHS is one of 5 similar cohort studies being conducted across Canada as the 300,000 subject, The Canadian Partnership for Tomorrow Project which is exploring the role of genetics, lifestyle, behaviour and the environment play in the development of cancer.

Considering the ethnic diversity of the province, including aboriginal communities, south and south east Asian peoples.
Recruitment procedures
Based on the 16 geographically dispersed communities across the province of Ontario which we have selected for the assessment centre sites, letters of invitation are sent to residents of the surrounding area within a 50 K radius. Addresses are accessed from an electronic, publically available data base. Letters invite potential participants to view the web site or contact the call centre for an appointment or more information. Letters are followed by a 3 weeks reminder post card .Those who book an appointment for an assessment receive an information package by mail and a 48 hr reminder call.

Collection procedures
Initial data collection takes place at a community based assessment centre. It includes self reported questionnaire on demographics, life style, some personal and family medical history; nurse interview on contra indications for physical measures or phlebotomy, BP, review of current medications and detailed medical history; physical measures including spirometry, bone fragility , BMI, etc; biospecimen collection blood ( or saliva) and urine. Post assessment data is collected by mailed questionnaire including physical activity, dietary information, occupational and residential history.
Follow up procedures
Follow up data collection has not yet been finalized. At this point we anticipate a short annual follow up by telephone, mail or email. Five year follow up of a sample of the participants which involves more comprehensive data collection and repeat specimen collection is planned pending funding .

We use an MS Access database for tracking the Laboratory processing of the bio samples within the Lab. It was written by a third party contractor in Visual Basic.
As of September 2009, we are in the process of taking the base components/functionality of GERT and tailoring it for the OHS requirements, for tracking bio samples from Assessment Centre to Laboratory to BioBank. We are still only on requirements gathering phase, it will be developed in Java and made available when complete.
