BACKGROUND

Entirely publicly funded, CARTaGENE will be an infrastructure for population genomics research. This resource could therefore contribute to the development of better diagnosis, treatment and prevention programs for disease. CARTaGENE seeks to create a resource for the advancement of genetic research, with the aim of improving the health of Quebecers. This public resource will operate under a governance framework and will consist of a databank and a biobank. CARTaGENE will contain environmental, demographic and health data. Its biobank will contain DNA and blood and urine samples. Access to CARTaGENE will be granted to researchers who are seeking to better understand how genes interact with other genes, with the environment and with lifestyle factors.

OBJECTIVES

CARTaGENE's objectives are:
1) To help researchers understand the genetic, environmental and lifestyle factors involved in common diseases such as heart disease, diabetes and cancer. This increased understanding of the determinants of health and disease will, in the long term, translate into improved disease prevention, diagnostics and treatment, and contribute to a better allocation of health care resources;
2) To contribute to the international harmonization of research tools and methods and governance approaches for population genomics studies. This will help to increase the statistical power and reliability of all population genomics studies, and to translate the studies into health benefits faster.

Supplementary informations

People living in Indian reserves and those usually living in the following institutional collective household are excluded of the study: hospitals, long-term facilities-based care, some prisons and correctional facilities, and some other facilities such as rehabilitation centres, therapy center or drug rehabilitation centre. Also, the Short Portable Mental Status Questionnaire (SPMSQ) is used over the recruitment interview (phone) to exclude people who can’t consent.

Recruitment procedures

The research team will call upon the population of Quebec. 20,400 individuals aged 40-69 years will be randomly selected from the index of the Régie de l'assurance maladie du Québec (i.e. the Quebec health insurance system). Selected candidates may accept or refuse to participate. Since the selection process will be random, the health status of the selected candidates will vary. The sample represents 1% of this age group in the selected regions of Quebec.

Collection procedures

Participants will be contacted by La Régie de l’assurance maladie du Québec (RAMQ). Consenting persons will answer a questionnaire; will meet a nurse, who will take various clinical measurements, blood and urine samples. Data and samples will be gathered through largely non invasive measures (weight, height, blood pressure, bone density, etc.) and carried out in a health care facility. The required time is approximately two and a half hours (2 hrs and 30 minutes).

Follow up procedures

Regular re-contact and follow-up via governmental electronic health databases.

Supplementary informations

During a first phase, 20,400 participants will be recruited. The first 400 will be randomly selected from the Sherbooke and Montreal metropolitan regions. These first 400 persons will allow CARTaGENE to finalize the protocols to be used for another 20,000. Recontact of the study participants will be done approximately 6 months after the baseline interview. Follow-up interviews will focus and assessment of nutritional status and environmental exposure and will not include biological sample collection.

Validity: The content of the questionnaire has been revised and approved by over 30 experts from various scientific fields. This questionnaire is largely harmonized with other large Canadian Cohorts (The Canadian Partnership for Tomorrow Project CPTP). A significant proportion of the questions included in the self-administered questionnaire were also part of the pilot phase questionnaire. which was tested in a sample of 223 respondants (age 40-69) The self-administered questionnaire has been piloted to test for time and general comprehension in a small group of people (n=8); average time for completion of the questionnaire was 4:32 minutes.

Bias: Items concerning residential history may be subject to recall bias

Information on validity

This questionnaire is largely harmonized with other large Canadian Cohorts (The Canadian Partnership for Tomorrow Project CPTP). A significant proportion of the questions included in the self-administered questionnaire were also part of the pilot phase questionnaire. which was tested in a sample of 223 respondants (age 40-69) The self-administered questionnaire has been piloted to test for time and general comprehension in a small group of people (n=8); average time for completion of the questionnaire was 29 minutes.
Scales that are included in this questionnaire have previously been validated and extensively used in different contexts, these include Patient Health Questionnaire (PHQ-9), General Anxiety Scale (GAD-7), Job Content Questionnaire (JCC), International PHysical Activity Questionnaire (IPAQ) (references are provided below).

Bias: The touch-screen application has been developped for use in the general population, including those individuals who are not familiar with computers. However, more testing is required to evaluate the degree to which the mode of administration may lead to response bias.

Limitations: The questionnaire is limited to people who can read and have not visual impairment.
